Coombs-negative hemolytic anemia is the clinical hallmark of a rare and potentially deadly disease called paroxysmal nocturnal hemoglobinuria (PNH).
Approximately 20% to 35% of patients with PNH die within 6 years of diagnosis despite historical supportive care. Although PNH is uncommon,18.6% (71/382) of patients with Coombs negative hemolytic anemia have been shown to test positive for a PNH clone.
